A blurry logo is almost always an image with the wrong resolution or an oversized file for its slot. Upload a PNG or JPG in the right size. The pass logo and the form logo are two different settings.

Steps

Set the pass logo in the template editor under Design & Content › Images.
Set the logo of the form separately in the form settings. Updating one does not update the other.

Check the image sizes reference for the pixel sizes of every slot.
